Insurance and Billing

Mt. Diablo Memory Center is a Medicare provider.  We will bill Medicare and your secondary insurance carrier for any services provided at our clinic. Medicare Advantage plans do require pre-authorization before rendering services in most cases. We will work with your medical provider to facilitate authorization.

We are contracted providers with John Muir Medical Group, Sutter East Bay Medical Foundation, Contra Costa County Health Plan, and Hill Physicians Medical Group.  Your referring physician must request pre-authorization for these insurance plans before scheduling services at our clinic.

We are not participating providers in any private insurance plans (e.g., PPO).  Services will be provided utilizing your out-of-network coverage.  We can assist you in determining the extent to which your insurance will cover our services. We also welcome cash-pay clients and accept payment by personal check, Zelle, or credit card; payment plans are available.
